Frequently Asked Questions

If you have FMLA forms that need to be filled out, we do require a fee for all forms that are filled out. The fee is as follows:

1-3 pages- $10.00
4-10 pages- $25.00

There will be a form the front office will have you sign. Please allow 14 business days for the completion of your forms. Please give yourself enough time as they are processed in order as received. We cannot process last minute forms on a rush basis! When dropping off your forms the fee is due at the time of turning it into the office. If a fee is not paid, unfortunately we will not be able to accept it and this will delay your process.

We understand there are times you must miss an appointment due to emergencies or other obligations. However, when you do not call to cancel an appointment, you may be preventing another patient from getting much needed treatment. If an appointment is not cancelled within 24 hours advance notice, you will be charged a $25.00 fee.

Due to the large block of time needed for surgery, if a surgery is not cancelled at least 5 business days in advance, you will be charged a fee of $50.00. These fees will not be covered by your insurance.

The newest guidelines recommend pap smears starting at age 21 for sexually active women and HPV testing starting at age 30. Most of the time, paps should be done every two years, unless you have new risk factors. However, your insurance does cover it once every calendar year from your last pap. Many women can stop getting peps after a hysterectomy with removal of the cervix or after age 65. The pap is only one test offered during the annual wellness exam and we recommend you be seen every year for a checkup.

We offer women’s wellness including well-woman care from adolescence to post-menopause. Menopause and hormone replacement therapy is available. We can prescribe and place all forms of birth control including birth control pills, the patch, vaginal ring, Nexplanon (implantable birth control), Mirena, Liletta, and Paragard IUDs, and surgical and non-surgical permanent birth control.

We offer options for the treatment of fibroids, endometrial ablation (Novasure), Myosure myomectomy, hysteroscopy, hysterectomy and other surgical procedures.

Phone and televisits require an appointment. Call the office at +1-623-547-7205 to schedule an appointment. Please see our Telehealth link for information on how to log on once an appointment has been scheduled. Insurance companies may apply a copay to your telehealth visit.

New patients can complete their registration and bring in the completed “New Patient Forms” (available on our portal) with them at their visit. Please complete the forms and print them out one sided to be scanned in. By filling out the registration forms before coming in, will help the registration process. All patients need to bring in a photo ID, insurance card, and their copay as it is due upon arrival.

New patients must arrive 20 minutes before their scheduled “check-in time” for completion of their registration.

If you are a new patient, please bring any relevant records ( referral, ultrasounds, test results, abnormal pap smear results, surgery records) with you if possible. You can also complete a Release of Medical Records form, but sometimes those can take a long time to be processed from other facilities.
